Term Life Insurance and Optional AD&D
Term Life Insurance is offered to eligible employees through two different vendors, UnitedHealthcare and Prudential.
During Annual Enrollment, UnitedHealthcare will allow current employee participants in the plan to increase their coverage by $30,000, up to a total of coverage amount of $500,000, without evidence of insurability. Employee coverage amount must be in $10,000 increments. If your spouse is currently enrolled, they can increase their coverage by $5,000 without evidence of insurability, not to exceed 50% of employee coverage amount or $100,000.
The new Workday election will replace any other application currently on file, so be sure to elect the entire amount of life insurance desired for you and your dependents effective January 1, 2017, as well as beneficiary designations.
Dependent child(ren) demographic information has not been required for life insurance enrollment in the past. LSU will now collect and track all dependent data and dependent verification forms (birth certificate, etc.) for Voluntary Life Insurance coverage in Workday. If your dependent is listed in your Workday account, no dependent verification document is required.
If you have dependent coverage and are not able to see your dependent listed in Workday, you MUST re-elect and submit dependent verification documents when you submit your election. Workday will automatically waive coverage if coverage is not elected or dependent information is missing.
If you are not currently enrolled, you can enroll during Annual Enrollment without medical underwriting for $10,000 employee coverage on a guaranteed issue basis.
If you would like to increase your coverage or apply for spouse coverage, you can apply for this benefit at any time throughout the year subject to medical underwriting. This plan provides guaranteed issue of coverage up to 5 times your annual salary or $500,000, whichever is less, if you enroll within 30 days of your benefits eligibility. Premiums are age-based and will adjust according to age.
The maximum amount you may purchase cannot exceed $1,000,000. Any amount purchased over $500,000 will require you to provide evidence of good health/insurability. If elected, your AD&D policy will be equal to the term life insurance amount.
Spouse coverage is available up to 50% of the amount of employee coverage, up to $100,000, if you enroll him/her within 30 days of your benefits eligibility or within 30 days from your date of marriage. AD&D coverage available in increments of $5,000, $5,000 minimum to $100,000, not to exceed 50% of the employees amount.
You may elect child(ren) Life Insurance coverage in the amount(s) of $5,000, $10,000, $15,000 or $20,000 for each eligible dependent child – no medical information required. Dependent Child(ren) are 14 days to 26 years of age. AD&D coverage available in increments of $5,000, $5,000 minimum to $20,000, not to exceed 50% of the employees amount.
*It is the employee’s responsibility to notify the Payroll Department to stop deductions when the youngest child is no longer eligible.

Accidental Death & Dismemberment (AD&D) Insurance
AD&D provides benefits due to certain injuries or death from an accident. The covered injuries can occur up to 365 days after the accident. The AD&D insurance pays certain percentages of the benefit amount based on the injury sustained. If elected, AD&D must match voluntary life elected amount. Terminating Employment/Portability forms
To cancel Term Life Insurance coverage, submit the Enrollment/Change Form and indicate cancel coverage.
If you are terminating employment (not retiring), but would like to continue your Term Life Insurance coverage with UHC, you may port coverage if you meet both of the criteria:
- In good health/not terminating employment due to health problems
- Under age 70
This coverage is offered by Prudential through payroll deduction for State of Louisiana employees and offers two plan options at a flat rate of $0.50 per $1,000 of coverage for both plans. Both of these plans allow you to continue coverage into retirement at the same rate.
The Basic Plan offers $5,000 of coverage.
The Basic + Supplemental Plan offers coverage of 1.5 times your salary up to $50,000.
Terminating Employment/Portability forms
To cancel your Term Life Insurance coverage with Prudential, you will need to use the Enrollment/change form and indicate that you would like to cancel your coverage.
